In 1969, a pharmaceutical chemist named Tu Youyou was recruited into a military research program in China so secret it was known only by a date: Project 523, named for the day it was founded, May 23, 1967. The Vietnam War was killing more soldiers on both sides through drug-resistant malaria than through combat in some jungle theaters, and Ho Chi Minh had asked Mao Zedong for help finding a treatment that still worked. Mao, in the middle of the Cultural Revolution, when most scientific research in China had been suspended and many scientists sent to labor camps, authorized a nationwide program anyway. Tu, trained in both Western pharmacology and traditional Chinese medicine, was put in charge of a team screening more than two thousand traditional remedies for antimalarial activity, working from ancient texts as much as from a chemistry bench.

Most of what they tested did nothing. One candidate, an extract from the sweet wormwood plant Artemisia annua called qinghao, showed promise in some tests and failed completely in others, with no obvious pattern to explain the inconsistency. Tu went back to the original source: a recipe from Ge Hong's fourth-century text Zhouhou Beiji Fang (Handbook of Prescriptions for Emergencies), which instructed soaking the plant in water, not boiling it, and drinking the resulting liquid raw. Every prior extraction attempt in the modern lab had used heat, the standard chemistry technique for pulling a compound out of plant matter, because heat generally increases extraction yield. Tu's insight was that the ancient method's insistence on avoiding heat was not a limitation of Han-dynasty technology. It was information. Whatever compound was doing the work might be destroyed by exactly the process meant to extract it.

Switching to a low-temperature ether extraction in 1971, Tu's team produced an extract that worked reliably against malaria in animal tests, every time. Before human trials could proceed through China's stalled regulatory apparatus, Tu and two colleagues tested the compound on themselves first, to establish it was safe. It was, and it worked in humans exactly as it had in mice. The active compound was isolated and named qinghaosu, artemisinin in English, in 1972.

Because Project 523 was classified, none of this reached the outside world for years. Tu published no author-credited paper on the discovery until 1977, and even then under a collective institutional byline rather than her own name, standard practice for the program. Western malaria researchers did not fully register artemisinin's significance until the 1990s, by which point chloroquine resistance had spread across Africa and Southeast Asia and the world badly needed exactly what Tu had already found two decades earlier. Combined with a partner drug as an artemisinin-based combination therapy, it is now the World Health Organization's recommended first-line treatment for uncomplicated malaria essentially everywhere the disease is endemic, and is credited with saving millions of lives since its adoption.

Tu Youyou had no doctorate, no medical degree, and had never worked or trained abroad, all facts Nobel committees and Western science journalism found unusual enough to comment on when, in 2015, she was awarded a half-share of the Nobel Prize in Physiology or Medicine, becoming the first mainland Chinese Nobel laureate in a scientific category and the first woman from China to win a Nobel Prize in the sciences. She was, by her own account, simply someone who had been assigned a problem during a period when the ordinary channels of scientific credit and international exchange had been shut down, and who had solved it by taking a fourth-century instruction more literally than any modern chemist had thought to.
